February Free Form - Blood Sugar Level Record

Valentines Day is coming up and that means indulging in sugary treats.  For those with diabetes and their caregivers, this can be a dangerous time.  Keeping track of blood sugar is very important and can help you avoid unplanned trips to the hospital. 
 
The Blood Sugar Level Record is useful when caring for a diabetic. Not all diabetics require testing as often as the chart allows (four times a day) but it is very handy when needed. Average the numbers by adding all readings in a day, then dividing by the number of times taken. There is room at the bottom for an average of all averages for the month. Option: Most glucose monitors average for 7, 14, and 30 days which will save you the hassle.

Caregiver Negotiations 101 - How to Get the Job You Want or the Care You Need

Having worked as a caregiver for more than 30 years in private care, adult care homes, and owning my own residential care facility, I have participated in the hiring process many times.  I have been on both ends of the hiring spectrum.  As an owner I hired employees and managed their work.  As a worker in various facilities, I have done hands on care under the supervision of my employers.  I have been self-employed and I have been employed by others.  In every case, the process of hiring or being hired was vital to my satisfaction in the partnership established.  This was due in large part to agreements  I negotiated ahead of time.

The tempting thing to do when hiring or being hired is to go with the emotions of how we feel about the person we are interviewing or being interviewed by.  Sometimes just having a job or hiring a worker seems like the most important goal.  A quick decision without much forethought or preparation will often quickly lead to misunderstanding and dissatisfaction.  This in turn leads to a continuous turnover of workers or another job search.  The time taken in initiating and training is wasted and must be done again and again with new workers or jobs.  The frustration and stress level increase with each failure to get it right.

Preparation ahead of time is invaluable when entering into a contract for caregiving.  Whether the position is for a relief caregiver, an hourly worker, a shift worker, or a full-time live-in companion/ caregiver there are many things to consider before entering into an agreement.  Negotiation ensures both parties are satisfied when the process is finished.  Working out all the details and answering all the pertinent questions ahead of time, guarantees a smoother working relationship with all parties involved.

Just the word “negotiation” can be intimidating for people who aren’t sure how to do it.  I have learned over time that the whole process can be simplified by taking the time to prepare for and anticipate as many areas as possible that need discussion and negotiation ahead of time.  An employer often wants the most work possible from an employee, for the least amount of output of resources and wages.  On the other hand, a person applying for work wants the most pay for a manageable amount of work done.  Often the gulf between expectations is very wide apart.  Many times only a fraction of the important issues are discussed. The mindset is usually that when the actual working relationship begins, these issues will be ironed out on the job. From experience I have found this to be a huge mistake.  It is much easier to establish ahead of time what you are able to do or allow, and what you absolutely will not give in on.  It’s a type of boundary and limitation setting that should take place in all relationships, personal and in employment situations.  Many problems can be avoided on the job when they have been discussed beforehand, in detail, with agreements made and understanding clarified.

A list of topics, issues, question, and expectations to check off during the interview will ensure every area is covered.   This will make you feel much more efficient and professional.  The process can actually be very satisfying when you know your future is being decided as you proceed.

You often hear advice given on how to dress and present yourself for an interview.  Those things are very important, but the art of negotiation will have a longer lasting impact on your ability to work in a friendly environment, with a staff that is considerate of each other and who all have consistent expectations.  Of course, one thing to remember is, the actual negotiations can only begin when you are confident you will be hired.

The Negotiation Checklist

I Remember, So You Don't Forget!

Memories are like little gems that can be passed on from generation to generation. As caregivers, we spend a lot of time with elderly loved ones and clients and often get to hear stories about thier lives and thier familes. The "I Remember" form is a place to write down those stories or other things you think might be important to the family.

This form is also great for personal use. Our lives are filled with moments worth remembering. Keep track of them now so you won’t forget them later. Just a short note to trigger your memory will give you the peace of mind that you will not lose that detail in the fog of the past. Do you want to write down the highlights of your life for future generations? How about the cute things you tell everyone your children or grandchildren said or did? Have you had any miracles or unexplained events impact you or those around you? Jot it down now and expand on it later. Even when you begin to actually write everything out this form will continue to be an ongoing gold mine of memories.

When the doctor gives new orders at doctor visits or over the phone, this form is especially helpful in keeping that medication information up-to-date. If an emergency arises and you have to rush to the ER or call an ambulance, you do not need to take your medication bottles with you if you take this form. The name of the medication, the dosage, directions, and the reason it is being taken are all documented on this form. Having all of the information organized on one simple form makes everyone's job easier, which helps ensure your client or loved one gets the best care possible.

Daily Work Schedules

Daily Work Schedule Outline: This form is the starting point for organizing your work schedule. List each thing that needs to be done daily, weekly, monthly, and yearly. Then use the Daily Work Schedule to accomplish those jobs.

Daily Work Schedule: This form is very handy for organizing your day. It actually works best when you fill out the companion form, Daily Work Schedule Outline, first and use it as a guide for each day’s entry. There are schedules for five days and for seven days. You won’t easily forget to do anything that needs to be done on a regular basis.

Wages Paid Forms

Record of Caregiver Wages Paid: If you are paying a caregiver this form is for you. Collect copies of pay stubs or canceled checks and enter details from each on its own line. Remember to keep all proof of wages paid with this form. This form will be the record of all wages paid to each caregiver for the year. Depending on how often wages are paid it may end up having more than one sheet. If you total them as you go in the Year to Date column, the final amount will go onto your Year-end Summary of All Wages Paid which will include all of the caregivers you have hired for the year and will be used for your taxes.

Record of Caregiver Wages Received: If you have received wages from a client or clients this form is for you. Remember to keep close track of all receipts for tax time. A suggestion is to put receipts in a plastic sleeve behind the form. That way everything stays together.

Year-end Summary of All Caregiver Wages Paid: If you have paid a caregiver or caregivers during the year this form is for you. The final step, at the end of the year, is to get a total of all the wages you have paid to prepare for taxes. Use one line for each caregiver who has worked for you during the year. Enter their information with total hours worked and the last amount from the caregiver’s Year to Date column on their own line. Add up the Total Amount Paid column to obtain the total of all wages you paid for the year. Simplifies everything!

Year-end Summary of All Caregiver Wages Received: If you have worked for a client or clients during the year this form is for you. Serial Numbers Record

You know you should! Everyone knows they should write down the serial numbers of the products they buy and store them in a safe place. This form will organize all your things on one (or more if you have lots of stuff) sheet of paper. Put a copy in your safe, give one to a trusted friend, put one in your safety deposit box at the bank, or where ever you think will be safe, just in case. If there is a fire, something is lost or stolen all you need to do is give a copy to your insurance agent. Information includes date, item/purchase price, serial number, and location/room. Ahhh, now you can relax.

Record of Contract Labor

This form keeps a record of all contract work you have had done. If you file to the front you can easily flip back through the pages to see details of the companies you have used in the past. Record of Birthdays and Anniversaries

Remember that sinking feeling when you know you have forgotten someone’s anniversary? Or you suddenly realize it’s a week past your sister’s birthday? This form will save you a lot of embarrassment. Just enter the name, date of birthday or anniversary, and e-mail or address. When the time is close enter what you chose, a card or e-mail card, and/or a gift. Product Preference Lists

Caregiver Product Preference List: You may have one or several clients to care for and over the years you have used many different products for their needs. Over time you have come to love and trust certain supplies for certain conditions more than all the others you have used. This form is where you can keep them all listed in detail so you will never have to struggle to remember what you used or need. With each new client transfer the products you want to use to their own Client Preference List (we have that one too) and start you job being efficient.

Client Product Preference List: Do you have to keep track of products for your client or several clients? Do you often forget just what you used in the past? When you are responsible for tracking purchases on a regular, or not so regular basis, or if you have several clients, it can be very confusing to get just the right product every time. This form is a great master list of everything you may require. Don’t try to remember, just look at your form. Need to make a list for current needs? Expense Records

Expenses, any kind of expenses, you need to track are made much easier with this form. It takes all the information you need to review and puts it on one page. Either way it is very important to have all your details in one place. Your tax man/woman will love you for sure! Even if you do the taxes yourself, this will save you a lot of headaches at the end of the year. Remember to separate the deductible, from the non-deductible, what you can claim now from what you can claim later at the sale of your property, and the labor you hired to help you. Don’t Bug Me Card

Asking health care professionals in the doctor’s office and the hospital to wash their hands is an embarrassing, intimidating challenge, and yet that is exactly what we are told to do! They all know it is a very important step they need to take between patients, but not enough of them do it automatically.
